Kaiser Permanente

The Kaiser plan is a health maintenance organization (HMO) that provides you and your covered family members health coverage and care delivery in one coordinated experience. 

This plan is only available to team members who live in Northern or Southern California.

How the HMO plan works

Primary care physician (PCP): You select your PCP when you enroll in the Kaiser plan, or Kaiser will assign one to you. Your PCP coordinates your health care with a team of doctors, nurses, specialists, and health educators within the HMO.

Preventive care: You pay nothing for preventive care, including screenings, immunizations, and exams.

Cost for care: There is no deductible. You pay a copay when you receive care.

Out-of-pocket maximum: Copays count toward the out-of-pocket maximum. Once you meet the out-of-pocket maximum, the plan pays 100% of covered services for the rest of the year.

No care outside the HMO: You must use Kaiser doctors and facilities, except in an emergency.

Prescription drugs: You pay a copay for generic and brand prescriptions at in-network pharmacies. Prescriptions filled at an out-of-network pharmacy are not covered.
