Set your sights on clear vision with coverage through VSP. Gen offers two vision plan options to choose from.
Plan highlights
Both the VSP 1.0 and VSP 2.0 plans cover eye exams, lenses, frames, and contact lenses at special rates.
You won’t receive an ID card; just identify yourself as a VSP member when you receive services from a VSP provider.
- Many frames and lenses are covered in full. Lens enhancements, which may be cosmetic in nature, may result in additional costs.
- You can receive 20% savings on additional glasses and sunglasses, including lens enhancements, from any VSP provider within 12 months of your last eye examination.
- Coverage allows for one pair of computer vision care glasses each calendar year for team members only—dependent coverage does not include computer vision care glasses.
- A retinal screening is included for an additional $20 copay during your routine eye exam.
- The VSP 2.0 plan includes laser eye surgery coverage. The plan pays $1,000 per eye per lifetime. VSP’s Laser VisionCare Program provides you with discounts for PRK, LASIK, and custom LASIK eye surgeries, with an average of 15% off, or 5% off if the laser center is offering a promotional price.

Benefits at Costco
The frame allowance at Costco is $115 (instead of $210) on the 1.0 plan and $135 (instead of $250) on the 2.0 plan.
Be sure to identify yourself as having VSP, so Costco or the optometrist will know to bill VSP directly. Otherwise, you will have to submit an out-of-network claim for reimbursement.
Only 66% of the optometrists at Costco are contracted with VSP, so before you go, be sure to find an in-network provider.
Benefits from other providers
If you visit a non-VSP provider, you can receive a reimbursement of up to $70 for frames and $30–$100 for lenses.
You must pay the provider in full and then file a claim for reimbursement through vsp.com.
No ID card required
VSP does not issue ID cards. Just tell the provider that you’re covered by VSP. You’ll need to provide your date of birth and your Social Security number.
